If you mistakenly entered an incorrect high school graduation date on your OSAP profile, you must submit one of the following:

• High school transcript.
• Letter from the high school indicating the date you last attended on a full-time basis (or is expected to finish full-time high school if the final year of high school is not finished).
• If you completed high school as a mature student, you must provide a transcript or a letter from the last high school you attended on a full-time basis prior to your return to high school as a mature student.

If you attended high school outside of Canada and are unable to obtain a transcript due to exceptional circumstance reasons, you can provide an Affidavit that states the following:

• The reason you are unable to provide a transcript
• The late date you attended high school on a full-time basis (graduation)
• The name/location of the high school
